FORM A - HCJFC APPLICATION FOR MEMBERSHIP / WAIVER AND RELEASE
FORM B - PLAYER REGISTRATION / PERMIT TO PLAY
FORM C - FOOTBALL VICTORIA PLAYER REGISTRATION / TRANSFER
New players - all new players are to complete FORM A and FORM B and provide a copy of proof of birth (birth certificate, passport etc) that needs to be retained by the club. Players who are transferring from another club also need to complete FORM C.
